Welcome to Lee and Frank Goldberg Family Religious School registration for 5785! We look forward to students from PreK-12th Grade joining us for an exciting and engaging year of learning, but first we must take care of a few administrative details.

Enrollment Eligibility Policy
Beth Israel Membership is required in order to enroll in Religious School (except for Pre-Kindergarten). Please contact the Religious School Office at 858-900-2551 or rstig@cbisd.org for more information.
Additionally, your Beth Israel account balance must be in good standing. You will be asked to choose a payment plan for tuition at the end of this form.

Every applicant will be charged a non-refundable registration fee of $125 per student upon submission of this form, which guarantees placement in the Religious School.

Please submit a separate form for each child enrolling.

* Emergency & Medical Information: Occasionally an emergency arises where it is necessary for a staff representative of Beth Israel to contact parents while their children are at school. Every effort will be made to notify the parents or someone designated by them if a child should become very ill or be involved in an accident. If this cannot be done, the policy of the Temple is to transport the child to the nearest emergency hospital. This action will be taken in all such cases unless instructions to the contrary are provided by the parents. I, the undersigned, parent/guardian of the aforementioned minor(s), do hereby authorize Beth Israel as agent for the undersigned to consent to any x-ray examination, anesthetic, medical or surgical diagnosis or treatment, and hospital care which is to be rendered under the general or special supervision of any physician or surgeon, who is licensed under the provisions of the California Medicine Practice Act, on the medical staff of a hospital, whether such examination, diagnosis or treatment is rendered at the office of said physician or at such a hospital. It is understood that this authorization is given in advance of any specific examination, diagnosis, treatment or hospital care being required and is given to provide authority and power on the part of our above-named agent to give specific consent to any and all such examinations, diagnoses, treatment or hospital care which the aforementioned physician in the exercise of his/her judgment may deem advisable. This authorization is given pursuant to the provisions of Section 25.8 of the Civil Code of California and will cover attendance at all authorized functions of the Religious School. I hereby consent to Beth Israel's emergency medical policy.

CLASS SELECTION

Please note that starting this year, 3rd-6th Grade Weekday Hebrew classes will be offered either online OR in-person.

In-Person classes will meet on Tuesdays at Beth Israel from 4:30-6:00pm.
Online classes will meet on Wednesdays on Zoom, from 4:30-5:10pm.
All 3rd-6th grade will continue to meet regularly on Sunday mornings from 9:00-11:30am.

3rd-6th Grade Weekday Hebrew class selections can be changed up until August 26. Please reach out to rstig@cbisd.org to swap sections.

PAYMENT

Please read this section carefully.

Today, you will be charged a $125 enrollment fee. This is a one-time, nonrefundable payment that guarantees your child's spot in the Religious School and marks your completion of this enrollment form.

If you selected a Madrichim-In-Training course, you will also be charged for the $180 tuition.

For regular grade-level tuition, you can choose one of the following options:

Pay in Full
Your full tuition will be charged to the payment method of your choice.

Pay in 2 Installments
Half your tuition will be charged to the payment method of your choice now. The other half will be charged on January 1st, 2025.

Pay in Monthly Installments
Your tuition will be broken up into equal monthly charges billed automatically between now and May 2025.

I'm Applying for a Scholarship
Only select this option if you have also checked the box to request a scholarship. Be sure to complete the separate scholarship form to follow. You will not be charged for tuition until an agreement has been reached.
